Shared Care Requests

As you may be aware, shared care guidelines are non-contractual (GMS)  work. Unfortunately, due to an increase in the GP workload, we are no longer able to safely undertake this work with our diminishing resources.

The Practices of Loxford PCN (Ilford Medical Centre, Ilford Lane Surgery, Loxford Practice, Mathukia Surgery and Oaktree Medical Centre) agreed not to take on any further shared care agreement as of 01/07/2024.

We will however continue to abide by the agreements, which we had previously signed up to for existing patients. We will also be unable to restart Shared care should a patient, who was previously on a shared care plan, have lapsed monitoring for a long period and therefore require re initiation and titration. We would request that you notify all members of your team and services of this new policy. We will be alerting our patients likewise.
